Pinned By Falling Metal Pallets, Man Killed At Berks County Manufacturing Facility: Coroner
A Schuylkill County man was killed in a workplace accident after being pinned by falling metal pallets at East Penn Manufacturing in Berks County.
Stephen Rapp, 48, of South Manheim Township, was pronounced dead at 8:41 p.m. on Tuesday, March 11, in the emergency department at Lehigh Valley Hospital–Cedar Crest, the Lehigh County Coroner’s Office said on Wednesday, March 12.
The fatal incident happened at 102 Deka Road in Lyon Station around 7:49 p.m. when the metal pallets fell on Rapp, causing mechanical asphyxia, officials said. His death has been ruled an accident.

A hazardous chemical gas release at a Chambersburg-based soap and detergent manufacturing facility has resulted in $161,310 in federal penalties, according to a U.S. Department of Labor announcement on Tuesday, Jan. 14, 2025.
The investigation, led by the Occupational Safety and Health Administration (OSHA) in its Philadelphia regional office, followed the July 2024 incident at AFCO, a manufacturing subsidiary of Zep Inc. based in Atlanta. Police ID Elizabeth Worker Crushed To Death At Clifton Recycling Plant (Update)
A worker was killed Tuesday afternoon, Nov. 12, when a baling machine fell on him at a Clifton recycling plant, officials confirmed.
Jose D. Centeno, of Elizabeth, went underneath the heavy machine at Ricova Recycling at 1 Broad St. around 3:30 p.m. and became crushed, according to a spokesperson from the Occupational Safety and Health Administration (OSHA) and Clifton police.
Clifton police Lt. Robert Anderson said officers, firefighters, rescue officials, and EMS personnel responded and confirmed Centeneo was dead.

A father and local business owner was crushed to death but not at his own business, Pennsylvania State Police explained to Daily Voice on Thursday, Jan. 18.
Jordan Dallas Zimmerman of Lewisburg, Lykens Township, was crushed by falling steel at Dryhill Manufacturing, located on North Crossroads Road, Trooper Kelly Abati told us.
