Woman Dies After Fall At PA Prison, Investigation Launched: DA Authorities are investigating after an inmate fell to her death at the Monroe County Correctional Facility, according to the District Attorney's Office.  The unnamed 42-year-old woman fell from an "upper tier" at the prison early Sunday morning, Feb. 4, and was badly injured, officials said. Kejuan Cooke was found unresponsive by his cellmate on Tuesday, police said.
