Woman Dies After Fall At PA Prison, Investigation Launched: DA Authorities are investigating after an inmate fell to her death at the Monroe County Correctional Facility, according to the District Attorney's Office.  The unnamed 42-year-old woman fell from an "upper tier" at the prison early Sunday morning, Feb. 4, and was badly injured, officials said.
