In September 2015, Cappelli brought on CEO Maria Trusa, who served as the executive director of Scarsdale Medical Group for 26 years.
Together, they also launched a new Medical Membership Program with a focus on providing benefit-rich, affordable care for the uninsured population of Westchester County.
This program officially launched March 15 and grants non-insured members access to affordable and dignified urgent care and physical therapy services for $1 per day ($365 per year). A membership fee schedule has been created that gives Formé’s members full transparency in regards to all costs associated with their medical care.
In addition, Formé has developed discount programs through its many partners. Members will receive approximately 50 percent discount off medical care, up to 89 percent off labs, up to 70 percent off on MRI, CT and Sonograms and up to 70 percent off prescription drugs.
“After many months of planning, we are thrilled to introduce this one-of-a-kind program that offers members of our community, with no health insurance, the opportunity to receive dignified medical care at our state-of-the-art facilities with no surprise bills," said Cappelli.
“It is of the utmost importance to us to work with other medical service providers in the area to extend services to the community at affordable prices,” added Trusa, a Tarrytown resident.
Formé, a certified Empire State Woman Owned Business is also in the midst of adding another 3,000 square feet for additional medical space.
"Since September 2015, Formé has been working on restructuring the company, building a strong foundation to support current business and prepare for future growth and additional locations," said Cappelli.
Formé is at 7-11 S Broadway, (914) 723-4900, www.formeurgentcare.com/.
