GOSHEN, N.Y. -- A nursing home in Goshen is being investigated by the state Department of Health. Sapphire Nursing and Rehab in Goshen is under investigation over staffing levels and its living conditions according to the Times Herald-Record. The company recently slashed its staff in half and has only 17 nurses. Rep.
