Man Charged In Fatal 1-95 Chain-Reaction Crash That Killed Franklin Grandmother A Rhode Island man has turned himself over to police after authorities said he was responsible for a Connecticut chain-reaction crash earlier this year that killed a 78-year-old woman.  Joseph Teixeira, 22, was charged with following too closely and misconduct with a motor vehicle, Connecticut State Police said.
