Jaleel Wright, who suffers from schizophrenia and may be depressed, went missing before and was found in Nyack, police said.
His family said they last saw Wright last Friday in Hackensack.
He was wearing a hoodie, wool jacket and baseball cap, all black, along with blue jeans and brown boots.
Authorities pinged his cellphone on Sunday to St. Nicholas Park off 135th Street.
Wright is 5-foot-9 and 130 pounds, with brown eyes.
He has a tattoo of a dog on his right wrist and a rocket on his right shoulder.
