The company — the world’s second-largest healthcare provider — announced the move to stockholders late last week as part of an enterprise-wide restructuring plan. Luis Hernandez, 26, and Youthana Inthirath, 54, were both arrested on June 10, police said. Hernandez allegedly swiped multiple items from CVS when he was confronted by an employee at the Fairfax Boulevard store, police said. Hernandez punched the employee multiple times, caused extensive damage to the store, and fled with stolen items, according to police. The winning numbers in that drawing were 1-4-6-6-1, and the Fireball number was 5.  While Choi didn’t match the five digits, he matched using the Fireball, which replaced one of the digits, to win $12,000 for each of those 50 plays.
