State police said the accident occurred in the southbound lanes between exits 10 and 11 in Clarkstown at 11:40 p.m.
Shmuel T. Nissen, 27, of Brooklyn, was operating a 2010 Toyota Sienna when a deer entered the roadway. Nissen attempted to take evasive action and swerved to the center median striking a tree, state police said.
The rear passenger, Yeshayahu Levy, 41, of Brooklyn, was ejected from the vehicle, causing serious head trauma, state police said.
The front seat passneger, Alexander Rosenblum, 32, of Brooklyn, pinned in the front passenger seat, had his right arm amputated below the bicep, and suffered a compound fracture of his left arm. Sgt./Station Commander Thomas Fortuna performed life saving measures on Rosenblum by placing a tourniquet on his right arm, said Trooper Steven Nevel.
Nissen was transported to Nyack Hospital, where he was treated and released. The two passengers were transported to Westchester Medical Center where they remain in critical condition.
