At 11:01 a.m., officers and paramedics responded to the 800 block of Grand Avenue after a bicyclist went over the handlebars while pedaling, Hackettstown police said.  A mechanical failure occurred in the bicycle's rear gear mechanism, causing the rear wheel to lock and the chain to derail from the gears, police said.
