Dr. Stanley Giudici, of Integrated Precision Medicine, had his medical license suspended by the New York Board for Professional Medical Conduct for “negligence on more than one occasion” and “failing to maintain accurate patient medical records."

The board claimed Giudici provided psychiatric treatment without properly seeing patients, or keeping proper patient records.

Giudici's patients were sent a letter dated Sept. 17, saying he was moving out of the area for family reasons and referred them to several doctors, according to recordonline.com.

This is the second time Giudici has been censored by the state. He had his license suspended by the same board in 2011 for prescribing drugs inappropriately to three patients without proper care, according to the state.

Following that action, the board said: "The physician is permanently restricted from ordering, prescribing, distributing or administering opiates/opioids."

Board records show the doctor completed the terms of the order on Nov. 28, 2014, and did not "contest the charge of negligence on more than one occasion."

An official with the Department of Health said the board doesn’t comment on individual cases.
