The worker was laying down new metal decking sheets using a screwgun on the roof of 125 North Street, near Huyler Street, when he slipped and fell around midnight Oct. 14 while trying to climb into an aerial lift that he was using as a power source, OSHA reported.
Hackensack University Medical Center EMS and paramedics responded -- along with Moonachie police -- and took him to the hospital, where he was pronounced dead soon after, responders told Daily Voice at the time.
A subsequent inspection out of OSHA's Hasbrouck Heights office led to the fines against Trinity Welding And Iron Works of Cookstown, in Burlington County.
