The Buick Enclave slammed the marked Toyota Sienna minivan on Woodland Avenue around 11 o’clock last night.
Responding to several 911 calls, police learned from witnesses that the driver had taken off.
The officers checked the Enclave’e registration and headed to the owner’s address — where they spotted the juvenile.
He bolted and ran through several backyards before being caught.
The Ridgewood Volunteer Ambulance Corps took him to The Valley Hospital nearby for treatment of unspecified injuries.
Assisting Ridgewood police at the crash scene were their counterparts from Glen Rock and Midland Park, as well as Bergen County Police.
Both vehicles were later towed by flatbed.
