The truck rammed the tree near Donny Brook Drive just after 9 p.m.
Forest Road was closed in both directions for more than an hour while emergency responders worked at the scene.
The victim was taken by basic life support ambulance to The Valley Hospital in Ridgewood with help from a hospital rig. A flatbed tow truck removed the vehicle.
Allendale police are investigating the crash.
