In her new role, Pelazza will conduct patient assessment visits at all area facilities throughout Fairfield County, build relationships with community and health-care centers and serve as the primary nurse for consultation, assessment and coordination of care for all hospice patients receiving inpatient care.
Pelazza has 20 years of clinical and administrative experience and recently received her certification from the National Council of Certified Dementia Practitioners, which is awarded to a health-care professional who has demonstrated competency and comprehensive knowledge in the area of dementia care.
Pelazza holds associates degrees in nursing from Excelsior College in Albany, in health science from Norwalk Community College and in general studies from Housatonic Community College.
