DANBURY, Conn. -- The 55-year-old man killed in a one-vehicle accident on Wednesday afternoon has been identified as a City of Danbury worker, according to a police statement released Thursday.

The man driving the utility truck was Samuel Crews, 55, of Danbury, police said.

At 5:07 p.m., the Danbury Police Department responded to Lake Avenue Extension for a motor vehicle accident. The accident took place just west of the Ethan Allen Inn.

A gray GMC Sierra utility truck for the City of Danbury traveling west on Lake Avenue Extension veered into the eastbound traffic lane, then continued to travel off the roadway and slammed into a tree, police said.

Crews was removed from the vehicle and transported to Danbury Hospital, where he subsequently died from his injuries.

The impact resulted in significant front-end damage to the vehicle.

Any witnesses are asked to contact Officer Lance Brevard or Sgt. Rory DeRocco at 203-797-2156.
