According to the Nassau County Police Missing Persons Squad, James Grant was last seen on Tuesday, Sept. 10 at noon, an incident reported Sunday, Sept. 22 at 1:50 p.m.
Detectives described Grant as a male black and said he suffers from a cognitive disorder and is need of medication.
He was last seen leaving Franklin General Hospital on Franklin Avenue, said police.
Grant is described at 5 feet 11 inches tall, weighing 180 pounds with brown eyes and multi-colored hair and beard.
His clothing description and possible destination are unknown.
