Clara Cares was designed in 2008 in response to the health care needs of medically underserved home bound seniors. For more information contact Fran Monteleone at 973-450-2997.
The program seeks to increase access to quality health care and provide effective disease management in order to prevent unnecessary hospitalizations and avoid acute illnesses through house-call health care services.
The Henry and Marilyn Taub Foundation provided generous support for this program. During facility visits, Clara Maass Medical Center personnel provide Primary Care Services, comprehensive medical assessments, follow-up counseling, and appointment/transportation coordination.
The Carucci Senior Building is located at 281 Stuyvesant Ave.
